Steps to arrange your consultation:

1. Do you have a referral from your PCP or Chiropractor? This is recommended but not required to proceed.

 

2. Type of insurance - Please confirm that our practice is in-network with your insurance.

 

3. If we are not in-network with your insurance we can still arrange your consultation, however, please look into your out of network coverage.

~We require an active Credit Card on file in the event your insurance denies your consultation.

 

4. Your insurance will determine if Botox is covered for medical necessity.

 

5. Once your insurance approves Botox for treatment, we will contact you to schedule your appointment after receiving confirmation that product has been delivered to our office.

 

6. We will submit the procedure to your insurance, even if we are out-of-network. In some cases, we may need to collect payment upfront and will submit the rendered payment to your insurance.

 

7. Any balance for non-covered procedures must be paid before scheduling your next session.
