STEPS:

1. Do you have a referral from your PCP - We need a referral to provide a consultation.

2. Type of insurance - Please verify our practice is in Network with your insurance.

3. If not in Network with your insurance- Fee due at time of Consultation $198. Claim will still be submitted to your insurance. 

4. Your insurance will determine if your Botox will be covered for medical necessity.

5. Once insurance covers your Botox, we will call you to set up your appointment.

6. We submit your visit and your procedure to our insurance. Your insurance will decide if your injections of the Botox was covered.

7. If you have a balance due to the practice for non-covered procedures, this will be due prior to rescheduling your next session. 

 

Botox for Migraine therapy is often every 3 Months and depends upon your Insurance Coverage.
