STEPS:

1. Do you have a referral from your PCP /Chiropractor?

2. Type of insurance - Please verify our practice is in Network with your insurance.

3. If not in Network with your insurance- Fee due at time of Consultation $198. 

4. Your insurance will determine if your Botox will be covered for medical necessity.

5. Once insurance covers your Botox, we will call you to set up your appointment.

6. We submit the procedure to your insurance.

7. If you have a balance due to the practice for non-covered procedures, this will be due prior to rescheduling your next session. 

 

Botox for Migraine therapy is often every 3 Months and depends upon your Insurance Coverage.
